Breast reduction removes breast tissue and skin, and repositions the nipple, to create smaller and higher breasts. Unlike most cosmetic surgery it often relieves genuine physical symptoms — neck, back and shoulder pain, shoulder grooving and skin irritation beneath the breast. Satisfaction rates are among the highest in aesthetic surgery.

Often a functional operation

Disproportionately large breasts cause real physical problems: chronic neck, upper back and shoulder pain, deep grooving where bra straps carry the weight, recurrent rashes and fungal infection in the fold, difficulty exercising, and difficulty finding clothes that fit. Reduction addresses these directly, and the relief is often immediate and substantial.

That functional element is why this operation has among the highest satisfaction rates in aesthetic surgery. Patients are usually solving a problem rather than pursuing an ideal.

The scars

Reduction requires removing skin as well as tissue, so scars are unavoidable and more extensive than for augmentation. The usual pattern runs around the areola, vertically down to the breast fold, and horizontally along the fold itself.

They fade and flatten over twelve to eighteen months but remain visible on close inspection. Most patients consider this an acceptable trade for the symptom relief, but you should see the expected pattern marked out before you consent.

Breastfeeding and sensation

Reduction can affect the ability to breastfeed, because ducts are divided during the operation. Some women breastfeed successfully afterwards and some cannot, and it is not reliably predictable. If future breastfeeding matters to you, say so at consultation — it influences technique and may influence timing.

Nipple sensation commonly changes. It usually recovers over months but can be permanently reduced or, less often, increased. Complete loss of nipple sensation is uncommon but possible.

Risks

Delayed healing at the junction of the scars is the commonest problem, usually settling with dressings. Altered or lost nipple sensation, asymmetry in size or nipple position, and unfavourable scarring including keloid formation all occur. Partial or, rarely, complete loss of the nipple due to inadequate blood supply is a serious complication and is substantially more likely in smokers, in very large reductions, and with certain techniques. Fat necrosis, infection, bleeding, seroma and blood clots are possible. Some women find the eventual size is not what they expected, in either direction.
