Rhinoplasty reshapes the bone and cartilage of the nose to change its appearance, and can be combined with septal surgery to improve breathing. It is among the most technically demanding cosmetic operations, swelling takes up to a year to fully settle, and a proportion of patients need revision surgery.

Function as well as appearance

Many people seeking nose surgery also have a breathing problem, often a deviated septum, and frequently have not connected the two. Assessing the airway is part of the consultation, and correcting a septal deviation at the same time makes sense where one is present.

It works the other way too: purely cosmetic changes can affect breathing if they narrow the airway, which is one of the reasons technique matters and why over-reduction of the nose is a problem rather than a bolder result.

Patience is part of the operation

The nose swells more than almost any other part of the face, and it swells for longer. Most swelling has settled by six to eight weeks, but refinement continues for a full year and at the tip sometimes longer.

This has a practical consequence: the nose you see at three months is not the final result, and judging the outcome early causes unnecessary distress. It also means revision surgery, if needed, is not usually considered until at least a year has passed.

Realistic goals

Rhinoplasty works with your existing anatomy — skin thickness, cartilage strength and bone structure all set limits. Thick skin conceals fine definition; thin skin reveals every irregularity. A nose can be refined and balanced with the rest of your face, but it cannot be swapped for someone else's.

Bringing a photograph of a different person's nose is common and understandable, and the conversation it prompts is a useful one. What we will do is tell you honestly which parts of that are achievable on your anatomy and which are not.

Risks

Dissatisfaction with the aesthetic result is the commonest problem, and this operation has a recognised revision rate higher than most. Breathing difficulty can occur or worsen. Persistent swelling, asymmetry, visible or palpable irregularities, and a small residual bump are all possible. Numbness of the nasal tip is usual early and occasionally persists. Bleeding, infection, and problems with the healing of the septum including perforation can occur. Where cartilage grafts are needed, they may warp or become visible over time.
